How do I schedule an appointment?
 
 
Crissman Family Practice prefers to see patients by appointment only.  We are fortunate to be able to accommodate and meet our patient’s needs by making appointments that same day or as much as a year in advance. Same day appointments are available Monday through Friday because we understand the often urgent need of our patients and their families when often faced with unexpected illnesses.

We offer the availability to register on line for a new patient account. The process is both fast and easy and will save you time when checking in for your appointment in our office.  We also have appointment staff available Monday thru Friday 8-12 and 1- 5 to assist you in making an appointment by phone.  Simply call our direct line at 336-226-2448 and we will be glad to assist you in making an appointment.

The appointment staff will ask you a few questions regarding your medical needs. The questions asked will allow us to appropriately access your appointment needs to make sure the amount of time is allowed with the health care provider. Giving thorough information when making the appointment will help us to provide the time needed to address your concerns.  If you have insurance, please have that information available when you are calling to schedule your appointment.  Please be aware that even though our practice provides care for a multitude of insurance companies, not all insurance companies will cover the care you receive at Crissman Family Practice. It is important that you check with your individual plan to make sure our practice is listed as a participating practice.

Please arrive 15 minutes early for your appointment in order to complete or update our patient information.  We ask new patients to arrive 15 minutes early to fill out required information.  The time needed to perform this task will be shorter for patients that enroll on line.   If you arrive fifteen minutes late for a regularly scheduled appointment or procedure you may be asked to reschedule at the physician's discretion.

If you must cancel an appointment, please call us at least 24 hours in advance to allow us to be able to offer that appointment to another patient in need.  If an appointment is not canceled within 24 hours a charge of $25.00 will be charged to your account.  We understand there are circumstances beyond your control so for special needs this charge may be waived at the discretion of the office staff or provider.
